Key takeaways
- Devoted Health is new to Putnam County for 2027, as part of its Greater Jacksonville service area.
- Humana, UnitedHealthcare/AARP and Florida Blue are all staying in the county.
- But several specific plans are terminating — two Humana Full Access Giveback PPOs and three Wellcare D-SNPs.
- A carrier remaining is not the same as your plan remaining. That distinction is the whole article.
- New doesn't mean better, and familiar doesn't either. Check doctors, drugs, pharmacy and out-of-pocket maximum.
- Devoted is new here, so provider participation has to be verified plan by plan.
Putnam County Medicare beneficiaries have a new Medicare Advantage carrier to consider for 2027: Devoted Health.
Devoted is expanding into Putnam County for the 2027 plan year, adding another option to a market that’s already seeing significant change. We’re ready to help Putnam County beneficiaries review Devoted plans as part of an overall 2027 Medicare review.
But a new insurance company arriving is only part of the story.
Several existing Medicare Advantage plans in Putnam County are changing or terminating for 2027, while other plans from familiar carriers are staying put. That makes this a year to look past the name of the insurance company on your card and review your specific plan.
Devoted Health expands into Putnam County
Devoted Health’s official 2027 plan information includes Putnam County in its Greater Jacksonville service area, alongside Clay, Duval, Nassau and St. Johns counties.
For 2027, Devoted lists multiple Medicare Advantage HMO options for this service area — traditional Medicare Advantage plans as well as certain plans designed for people who meet specific Medicaid or chronic-condition eligibility requirements.
That’s a genuinely new option here. But as with any Medicare Advantage plan, availability in your county doesn’t make it a good fit for you. Before enrolling, compare the provider network, prescription drug coverage, pharmacy costs, copays, maximum out-of-pocket and other benefits against your actual healthcare needs.
Which carriers are staying in Putnam County?
Devoted’s arrival doesn’t mean the others are disappearing. Based on the 2027 plan information available so far, Putnam County continues to have Medicare Advantage options from:
| Carrier | Status for 2027 |
|---|---|
| Humana | Continuing |
| UnitedHealthcare / AARP Medicare Advantage | Continuing |
| Florida Blue / BlueMedicare | Continuing |
| Devoted Health | New to the county |
There are also different plan types depending on eligibility: HMO, PPO, Dual Eligible Special Needs Plans (D-SNPs) and Chronic Condition Special Needs Plans (C-SNPs).
Some Putnam County plans are terminating
According to the 2027 Medicare Advantage plan data we’ve reviewed for Putnam County, several existing plans are listed as terminating.
Humana
| Plan | Putnam County members enrolled |
|---|---|
| Humana Full Access Giveback H5216-393 (PPO) | ~1,347 |
| Humana Full Access Giveback H7617-111 (PPO) | ~649 |
Wellcare — several Special Needs Plans are also listed as terminating:
- Wellcare Dual Access Sync (HMO D-SNP)
- Wellcare Sunshine Health Dual Align (HMO D-SNP)
- Wellcare Dual Reserve (HMO D-SNP)
That doesn’t mean Humana or every Wellcare option is leaving Putnam County. It means these specific plans are identified as terminating.
Humana is still in Putnam County
We want to be emphatic about this, because Medicare information gets garbled quickly during Annual Enrollment.
Humana still has Medicare Advantage plans in Putnam County for 2027. The 2027 landscape data we reviewed shows continuing Humana options including Humana Gold Plus plans, certain Humana Giveback plans and several Special Needs Plans.
So if you hear that a Humana plan is terminating, don’t assume Humana is leaving. The question is:
Is my specific Humana plan continuing for 2027 — and if it is, how is it changing?
The same principle applies to every carrier. We wrote about the wider picture in Humana’s 2027 plan cuts.
UnitedHealthcare and Florida Blue are staying too
UnitedHealthcare continues to have options in Putnam County for 2027. The landscape information includes continuing AARP Medicare Advantage from UnitedHealthcare HMO-POS and PPO options, plus certain UHC Dual Complete plans for people who qualify for both Medicare and Medicaid.
The 2027 data also shows BlueMedicare Select (PPO) remaining in the market.
So Putnam County beneficiaries keep multiple carriers and gain Devoted as a new one. More choice can help — it also makes comparison harder.
Why your Annual Notice of Change matters
If you already have a Medicare Advantage plan, the most important document you get this autumn is your Annual Notice of Change (ANOC). Plans send it each fall to explain changes to coverage, costs and plan details taking effect in January.
Don’t throw it away. Your plan may still exist in 2027 but have different:
- Doctor or hospital networks
- Prescription drug coverage
- Copays and coinsurance
- Maximum out-of-pocket costs
- Pharmacy arrangements
- Prior authorization requirements
- Dental, vision, hearing or other supplemental benefits
A plan that worked well in 2026 can still be available in 2027 and work differently. Our walkthrough of how to read the ANOC covers the sections worth checking.
What if your plan isn’t renewing?
Don’t panic — and don’t ignore the notice either.
A plan termination means you need to understand what’s happening to your coverage and what options you have for the new year. That’s different from simply deciding you’d like to shop around; your options and enrollment rights can depend on the circumstances surrounding the termination.
Which is why we recommend reading the actual notice you received rather than relying on a friend, a Facebook group, or someone else’s Medicare situation.
Should you switch to Devoted because it’s new?
Not necessarily. New doesn’t automatically mean better, and familiar doesn’t either.
When we review Medicare Advantage options, we look at the things that actually affect how you use the insurance:
- Doctors and specialists — participating with the plan for 2027?
- Hospitals — are the systems you want in network?
- Prescriptions — on the formulary, and at what cost?
- Pharmacy — preferred, standard or out of network?
- Medical costs — copays for the services you actually use
- Maximum out-of-pocket — your exposure in a difficult health year
- Plan rules — referrals, prior authorizations, other requirements
Extra benefits can matter. They shouldn’t be the only reason someone picks a plan.
“Do my doctors take Devoted Health?”
Because Devoted is new to Putnam County, this will be the biggest question — and it has to be checked individually.
Devoted provides a 2027 provider-search tool, but participation should be verified for the specific plan being considered. Same approach with any carrier.
Frequently asked questions
Is Devoted Health available in Putnam County for 2027? Yes. Devoted’s 2027 information includes Putnam County in its Greater Jacksonville service area, along with Clay, Duval, Nassau and St. Johns.
Is Humana leaving Putnam County? No. Specific Humana plans are listed as terminating, but Humana continues to offer Medicare Advantage plans in the county for 2027.
Which Putnam County plans are ending? The data we reviewed lists two Humana Full Access Giveback PPOs (H5216-393 and H7617-111) and three Wellcare D-SNPs.
My plan is terminating. What happens to my Medicare? You don’t lose Medicare eligibility. You do need to decide how to receive coverage from January 1, and your enrollment rights can depend on the circumstances of the termination.
Should I switch to Devoted because it’s new? Only if it fits your doctors, prescriptions, pharmacy and costs better than the alternatives. New isn’t automatically better.
Do my doctors accept Devoted? Verify for the specific plan. A practice accepting Medicare, or another product from the same insurer, doesn’t establish participation.
What if my plan is continuing — do I still need to review it? Yes. A continuing plan can change its network, formulary, copays and out-of-pocket maximum. That’s what the ANOC tells you.
Putnam County has more to review this year
Devoted Health gives Putnam County another carrier for 2027. At the same time several existing plans are terminating, others are continuing, and beneficiaries may see changes to costs, benefits, networks and prescription coverage.
This is not a year to look at the insurance company name on your card and assume nothing changed. Check the exact plan.
If you live in Palatka, Interlachen, Crescent City, Welaka, Pomona Park or elsewhere in Putnam County, we can help you review the 2027 Medicare Advantage landscape — including the new Devoted options — alongside the other Medicare coverage available locally.
Our goal isn’t to put everyone with the same insurance company. It’s to look at your doctors, hospitals, prescriptions, pharmacy preferences, costs and coverage needs so you can decide for yourself.
